1. Passport form, duly filled out over the internet (on line), printed and signed.

2. One photograph taken within the past six months, full face, front view, without glasses or head covering, white background, two inches by two inches (the standard size for a US passport).

3. Payment of the fees listed below, in U.S. Postal Service money order (sold at the U.S. Postal Service branches). Neither cash, checks nor credit cards are accepted.

- US$  80.00 for a regular passport
- US$160.00 to replace a lost, damaged or stolen passport

(The above amounts are for the new biometric model passport)

Those over 18 years of age must show:

4. An expired passport or a passport about to expire (original document).

5. Proof of fulfillment of all voting obligations. Please visit www.tse.jus.br and print out the receipt showing your status. In case of your situation is not regular, the passport WILL NOT be issued. You must come in person to a Brazilian Consulate to update your voting situation.

6. Military document (only males from 18 to 45 years of age).

Those under 18 years of age must show:

8. Birth certificate issued in Brazil or by a Brazilian consular authority abroad (the original or a photocopy).

9. Authorization to issue the passport, signed by both father and mother:

• If the minor’s parents (father or mother) or legal guardians are Brazilian, both of them must sign an Authorization form at the Consulate for a waiver of the consular legalization fee. If only one parent or only one legal guardian comes in person, the signature of the other parent or legal guardian who is not present must be notarized by a Notary Public before submitting it to the Consulate.
• In any case, if one of the minor’s parents (father or mother) or legal guardians is Brazilian, the non-Brazilian citizen’s signature must be notarized by a Notary Public before the Authorization form is submitted or mailed to the Consulate.
• If the Authorization form is mailed, the signatures of both the father and mother (or of both guardians) must be notarized by a Notary Public even when both parents, or both guardians, are Brazilian.
• Brazilian parents must show proof of Brazilian citizenship (a passport, an identification card, or a military document) when they come in to the Consulate. If the documentation is mailed, notarized copies of the documents must be sent.

Previous passports which have been lost, damaged or stolen

10. Lost or damaged passports: the applicant must sign in the presence of a consular agent a declaration explaining how the passport was lost or damaged and declaring this to be the truth.

11. Stolen passports: the applicant must bring in the original police report stating that the passport was stolen.

12. Whenever a passport is lost, damaged or stolen, the applicant must submit proof of Brazilian citizenship – a Brazilian birth or marriage certificate, a Brazilian identification card, a Brazilian military document, and/or any other documents deemed necessary by the Consular authority.

Important Notes:

A. Last name changed due to marriage

The person must show the Marriage Certificate indicating the married name. If the certificate is not from Brazil, the marriage must be previously registered at a Brazilian Consulate. If the applicant does not provide a Marriage Certificate indicating the new married name, the last name on the new passport will continue to be the name on the previous passport or birth certificate.

B. Foreign-born children of Brazilian parents must present a Brazilian birth certificate issued in Brazil or by a Brazilian Consular Office. If the child has not being registerd with the Consulate, the parents must do that first, before they can apply for the Brazilian passport. The registration bust be done previously and cannot be done on the same day of the passport application.

C. The required documents must be submitted or mailed either in their original form or as certified copies.

D. Processing time is 15 business days, starting only when all necessary documentation has been received.

E. Applications for passports may be submitted by mail as long as the applicant lives under the jurisdiction area of the Consulate. The applicant must send the documentation in a United States Postal Service Express Mail envelope. Post office Express Mail service provides a tracking number and it is faster. For those who come to summit the application in person, they will be helped on a first come first served basis. The Consulate do NOT schedule appointments for passport issues.

• The validity of passports submitted by mail will be of no longer than three years, since it will not be possible to collect fingerprints.
• Passports submitted by mail will be activated before mailed back. In case of any mistake it will be necessary to submit a new application and pay all the fees again.
• If the applicant wants the passport returned by mail as well, this person must enclose a post office Express Mail stamped self-addressed envelope (for sale at all United States Postal Service branches) inside the envelope sent to the Consulate.
• Effective January 1, 2008, envelopes from courier services such as Federal Express, UPS and DHL will no longer be accepted. The Consulate General will only accept envelopes shipped by the United States Postal Service (Post Office). The same rule applies to envelopes for returning documents: only pre-paid (stamped) United States Postal Service envelopes will be acceptable for doing so.
